Why ACL Injuries Are More Common in Women Skiers, and How to Come Back Strong

The “pop” is the sound every skier dreads. It is the moment an ACL tears, and for many skiers, it means an abrupt end to the season.

For women, this injury is especially common. Research across sports has repeatedly shown that female athletes have a higher risk of ACL injury than male athletes in sports that involve cutting, pivoting, landing, speed, and sudden changes of direction. Depending on the sport, women are often reported to be two to eight times more likely to tear an ACL than men in comparable activities.

Skiing adds its own layer of complexity. Speed, fatigue, variable snow, twisting falls, backward falls, and equipment that does not release in every possible injury scenario can all contribute. The result is a perfect storm: a sport women love, combined with movement patterns and forces that can put the knee in a vulnerable position.

The good news? Women are not “built wrong.” ACL risk is not about weakness. It is about anatomy, biomechanics, hormones, strength, neuromuscular control, fatigue, sport exposure, and equipment all interacting at once. And while you cannot change every risk factor, you can understand them, train around them, and return to skiing with more confidence.


Why Are Women More Likely to Tear an ACL?

ACL injuries are almost never caused by one single thing. They happen when force, body position, timing, and sometimes bad luck line up in the wrong moment.

For women, several factors may increase the risk.

1. Pelvis Width, Q-Angle, and Knee Position

One of the most commonly discussed anatomical factors is the Q-angle, which refers to the angle between the hip, kneecap, and shin. Because women generally have a wider pelvis, the femur tends to angle slightly more inward toward the knee. This can increase the tendency for the knee to move into a valgus position, where the knee collapses inward relative to the hip and ankle.

In skiing, that matters because the knee is constantly managing pressure from the ski, boot, binding, terrain, and gravity. If the knee falls inward while the ski edge is engaged, or while the body is rotating, the ACL can be placed under high stress.

This does not mean every woman has a risky alignment. It means many women benefit from strength and movement training that reinforces hip control, glute strength, hamstring strength, and knee tracking.

2. Neuromuscular Control and Landing Mechanics

Many ACL injuries are non-contact. That means the injury happens without another person crashing into the athlete. Instead, the knee gets overloaded during a landing, pivot, deceleration, or awkward recovery movement. General ACL literature notes that a large share of ACL injuries occur without direct trauma.

Women, on average, may land or decelerate with different mechanics than men. Some common patterns include:

  • Knees drifting inward
  • More upright torso position
  • Less hip and knee flexion
  • Greater reliance on quadriceps versus hamstrings
  • Less dynamic control through the hip and trunk

In skiing, this can show up when a skier gets in the backseat, catches an edge, lands off balance, or tries to recover from a fall with the ski still attached. The ACL is especially vulnerable when the knee is twisted, the ski is loaded, and the muscles cannot react quickly enough to protect the joint.

3. Quad Dominance and Hamstring Strength

The quadriceps and hamstrings work together to stabilize the knee. The quadriceps help extend the knee, while the hamstrings help control forward movement of the tibia. Since the ACL also helps prevent the tibia from sliding too far forward, strong hamstrings can play an important protective role.

Some women are more quad dominant, meaning they rely heavily on the front of the thigh instead of sharing the load through the glutes and hamstrings. That pattern can increase stress on the knee, especially during hard turns, landings, and sudden stops.

For skiers, this is why ACL prevention work often includes hamstring strength, glute strength, single-leg stability, core control, and balance training. It is not just about being strong. It is about being strong in the positions skiing actually demands.

4. Hormonal Influences and Ligament Laxity

Hormones may also play a role. Estrogen, progesterone, and relaxin can affect connective tissue, ligament laxity, and neuromuscular control. Some research has explored whether certain phases of the menstrual cycle may be associated with increased ACL risk, though this is still a complex and evolving area of study.

The takeaway is not that women should be afraid of activity during certain cycle phases. It is that women’s physiology deserves more specific research, more thoughtful training, and more individualized care. For too long, sports science was built around male athletes as the default. That is changing, but there is still a lot to learn.

5. Fatigue, Conditions, and Decision-Making

ACL injuries often happen when the body is tired, and the reactions are just a little slower. Late afternoon laps, sticky spring snow, flat light, chopped-up terrain, and “one more run” decisions all matter.

Fatigue changes movement quality. The hip drops. The knee caves. The skier gets backseat. Reaction time slows. A small mistake becomes harder to correct.

For women returning from an ACL injury, this matters even more. Your first days back should be short and intentional, not full-send bell-to-bell days. There is no award for proving you are fine on day one.


The Mental Side of an ACL Injury

Returning to skiing after an ACL tear is as much a mental challenge as it is a physical one.

Recovery is a long road, often requiring six months to a year of rehabilitation to regain strength, mobility, and confidence. For many skiers, the hardest part is not the gym work. It is trusting the knee again on snow.

The sound, the fall, the moment it happened, the fear of doing it again. Those memories can stay in the body. Even when your surgeon, physical therapist, and strength testing say you are ready, your brain may still hit the brakes.

That is normal.

Confidence comes back gradually. It usually does not arrive all at once. It returns in small moments. The first easy groomer. The first turn that feels natural. The first day you stop thinking about your knee every second. The first time skiing feels fun again instead of like a test.

Skiing on a blue run. My tip just sort of dug into some heavy snow, and I overextended my knee. It was not the classic backward twisting fall situation. 
What was the hardest or most surprising part of recovery? 
How quickly you lose your range of motion. Before surgery, I could still move around just fine. Then to go from being able to walk to not being able to walk, that was a bummer and sort of discouraging. 
What would you tell another woman going through it?
It will get better! It seems a daunting recovery process at the beginning, but it does get easier. The first year back on skis might not feel like your strongest year, but you'll get back to your pre-injury skiing level with a bit of patience

I tore my ACL skiing from Chair 8 to Chair 26 on a bluebird day at Vail Mountain. 

What was the hardest or most surprising part of recovery?
The hardest part was how long the recovery took. I was able to ski again the following season, but it took several years before I felt completely back to normal doing other activities, especially running and yoga.

What would you tell another woman going through it?
Do all the physical therapy, and then do more. It makes an enormous difference in your recovery, your range of motion, and your ability to return to skiing and all the other activities you love. It can feel slow, but the work you put into recovery really pays off.

What was the hardest or most surprising part of recovery?

The length of time and dedication it takes to heal. You hear statistics that ACL injuries are common. While true, that does not equate to easy. I was very surprised by the length (2 years) and complexity (I had meniscus damage) of the recovery, as well as the dedication required to maintain physical therapy.

What would you tell another woman going through it?

Everyone’s injury is different. While this may be a common injury, there are many variables. You will get better, but it will take time. Give yourself that time. Stay in physical therapy for as long as possible. Talk to the therapist to be sure you are performing the exercises correctly and that all muscles are firing. You will have a long relationship with this person. Don’t be afraid to change physical therapists if you’re not getting support. 

One of the biggest takeaways we've had at ourdoor DIVAS, while an ACL injury may interrupt your season, and alter your physical activities for a period of time, it doesn’t have to end your relationship with skiing. 


How to Return to Skiing After an ACL Injury

Getting back on skis after this type of injury is a physical and mental challenge for even the most seasoned athletes. Rehab builds strength, but no amount of gym work can perfectly recreate the unique stresses of skiing.

The best approach is gradual, humble, and smart.

Start Easier Than You Think You Need To

If you were an expert skier before your injury, start on easy blue cruisers. If you were an intermediate skier, start on mellow greens. Choose terrain where you can move smoothly, stay centered, and make predictable turns.

This is not a step backward. It is a smart re-entry.

The first few days are about collecting information:

  • How does your knee feel?
  • How does it respond after skiing?
  • Are you bracing or moving naturally?
  • Are you getting tired faster than expected?
  • Are you skiing defensively?

Keep those first days short. Ski a few runs. Take breaks. See how your knee feels that night and the next morning. Then build from there.

Avoid the “Prove It” Trap

It is tempting to go straight back to the terrain you loved before the injury. Steeps, bumps, trees, powder, speed, all of it.

But early return-to-ski days are not the time to prove anything. They are the time to rebuild trust.

Let your confidence catch up to your strength. Let your timing return. Let your body remember skiing without panic.

Work With a Coach or Instructor

A lesson can be incredibly helpful after an ACL injury. Not because you forgot how to ski, but because a good instructor can help you identify defensive movement patterns, backseat habits, knee collapse, or asymmetries that may have developed during recovery.

Sometimes the body protects itself by creating new habits. A coach can help you unwind those before they become permanent.


Should You Change Your Ski Equipment After an ACL Injury?

It is completely natural to question your equipment after an ACL injury.

You might wonder:

  • Did my ski hook up unexpectedly?
  • Did my binding fail to release?
  • Was my DIN too high?
  • Was my ski too demanding?
  • Was I on the wrong setup for the conditions?
  • Would a different binding help me feel safer?

These are valid questions.

No ski binding can guarantee prevention of every injury. Even a correctly adjusted binding cannot release in every possible fall or twisting scenario. Binding function depends on many variables, including skier type, height, weight, boot sole length, DIN setting, fall direction, snow conditions, and the forces involved.

That said, equipment can play a role in confidence and risk management.

A good comeback ski setup should:

  • Be familiar enough to trust
  • Be forgiving enough to reduce anxiety
  • Be stable enough to avoid feeling twitchy
  • Match the terrain you are actually skiing
  • Support your current strength and confidence level

Your first season back may feel different, but many skiers are surprised by how quickly they return to skiing “normally” on the terrain they once enjoyed.


What Helps Reduce ACL Risk?

There is no way to make skiing risk-free, but there are ways to stack the odds in your favor.

Strength Training

Focus on:

  • Hamstrings
  • Glutes
  • Quads
  • Calves
  • Core
  • Single-leg strength

Skiing is a single-leg sport hiding inside a two-leg sport. The stronger and more balanced each side is, the better.

Neuromuscular Training

This means training your body to control alignment, absorb force, and react quickly.

Helpful work may include:

  • Balance drills
  • Lateral hops
  • Controlled landing mechanics
  • Agility work
  • Plyometrics
  • Deceleration drills
  • Sport-specific conditioning

ACL injury-prevention programs that include neuromuscular training, strength, balance, and landing mechanics have been shown to reduce risk in female athletes.

Ski-Specific Conditioning

Before ski season, add movements that mimic skiing:

  • Lateral lunges
  • Skater hops
  • Step-downs
  • Wall sits
  • Split squats
  • Rotational core work
  • Eccentric quad and hamstring work

Your body should not meet ski season for the first time on opening day.

Smart Skiing Choices

Your choices on the mountain matter too.

Consider:

  • Warming up before skiing
  • Taking breaks before fatigue takes over
  • Avoiding “one more run” when tired
  • Choosing terrain that matches your current confidence
  • Being extra cautious in heavy snow, flat light, and end-of-day conditions
  • Getting your bindings checked every season

This is not fear-based skiing. It is intelligent skiing.

An ACL injury can feel like a hard stop. But for many women, it becomes a turning point.

You learn your body. You learn your strength. You learn patience. You learn that confidence is not something you either have or do not have. It is something you rebuild.
